A Clinical and Statistical Study of the Elderly Primiparas

Abstract
The Elderly Primipara is generally defined as a woman at the age of 35 years or greater if she is experiencing the birth of her first viable baby. Numerous report have stressed the decline of physiological activity and efficiency in this group of patients, coupled with an increased incidence of complications giving rise to high mortality and morbidity in both mother and fetus.
The elderly primiparas from October 1980 to April 1983 in the department of Obstetrics and Gynecology in Seoul National University Hospital were reviewed.
The results obtained are as followings:
1. There were 45 cases among total 4825 deliveries and the incidence of the elderly primipara was 0.93%.
2. Maternal age was distributed from 35 to 44 year-old.
3.. Concerning the antepartal complications, the incidence of PROM was 15.6% comparing with 13.3% in the control group. The incidence of hypertensive disorder was 8.9% comparing with 6.7% in the control group.
4. 41 cases (91. 1%) of total cases were of Vertex presentation and breech presentation was in 3 cases and transverse lie in 1 case.
5. 21 cases (46. 7%) delivered vaginally and 24 cases (53.3%) abdominally.
6. Among the indications of Cesarean section, the highest incidence was elderly primipara, other indications were fetal distress, CPD, breech presentation, and transverse lie with placenta previa in order of frequency.
7. There was no perinatal death and no congenital malformation.
8. The incidence of spontaneous abortion was 15 cases (33.3%) comparing with 5 cases (11. 1%) in the control group.
9. The mean gestational age was 38}6 weeks comparing with 40 weeks in the control group.
10. The mean birth weight of the infant was 3035gm comparing with 3137gm in the control group. The incidence of Apgar score above 7 in five minutes was 42 cases (93.3%).
